When you contact us by phone to schedule your first appointment, our staff can email you a secure link to the forms. This link will take you to a secure email form request where you can enter your basic information, such as your First Name, Last Name, Date of Birth (DOB), Email Address, and Phone Number. Once you submit this information, you will receive an email with a secure link to the forms. You or your alternate contact can then fill out the forms online. This method is the fastest and easiest way to complete the required forms.

Method 2: Inline Form Submission

If you prefer to fill out the forms directly on our website, we offer an inline form submission option. Simply locate the inline form below that contains both the Client Case History Form and the Acknowledgment of Review of Notice of Privacy Practices. You can fill out the fields and sign in both places required using your desktop mouse or touchscreen. This method provides a convenient way to complete the forms without the need for additional downloads or emails.

Method 3: Download Fillable PDF Form

For those who prefer to work with a downloadable form, we provide a fillable PDF option.
